• Government ID: Aadhaar Card, Driving License, Voter ID, Passport.
• Passport & Visa: Foreign nationals.
• Passport size photos: 2–4 photos for permits and registration.

2. Medical and fitness

• Medical certificate: Fitness certificate signed by a certified MBBS doctor.
• Personal health: Accurate information about pre-existing conditions (e.g. asthma, IBS, diabetes).

3. Personal and contact details

• Contact: Full name, age, gender, mobile number.
• Emergency contact: Name and number of a family member.
• Physical stats: Height and weight (BMI check required for some treks).

4. Booking and payment terms

• Advance payment: A portion (e.g. 30%) of the total fee to confirm the slot.
• Trek insurance: A copy may be requested for high-altitude treks.

5. Further requirements

• Minors (15–18): Must be accompanied by a parent/guardian or have a signed consent form.
